TRANSCRIPTS AND ACADEMIC RECORDS

 

Academic Records

 

The Temple School of Medicine Office of Student Records (OSR) is responsible for maintenance of academic records for current medical students and medical alumni.  Services for enrolled students include oversight of grade posting and grade changes, processing of loan deferment forms, replacement of missing photo identification cards and transmittal of official documents.

 

Graduates of the medical school should contact the office for assistance in processing, certifying diplomas and other documentation required for licensure or employment. 

 

 

Transcript Requests

 

Official transcripts for alumni and students are issued only upon request and are sent directly to another educational institution, state agency or potential employer. Temple University has authorized the National Student Clearinghouse to manage transcript ordering via the Web. To order a transcript, log onto the Main Campus Office of the Registrar website at http://www.temple.edu/registrar/ and click on the link for Transcripts; then follow the link for the Clearinghouse. The cost per transcript is $8.25, which includes a Clearinghouse processing fee. (The School of Medicine Records Office is no longer able to process requests for transcripts.)

 

 

Enrollment Verifications

 

Students and alumni seeking enrollment verifications can obtain them via the National Student Clearinghouse, accessible on the Temple University Registrar’s websiteat http://www.temple.edu/registrar/.

 

 

Document Requests

 

Current medical students and medical alumni requesting documents other than transcripts may pick up a Document Request Form in the medical school’s Office of Student Records, Medicine Education and Research Building, Room 328, Monday through Friday.  The form can also be downloaded here:

There are no charges for enrollment verifications, certified diplomas, copies of Dean’s Letters and other forms processed in the medical school’s Office of Student Records.  Fees for transcripts are handled by the National Student Clearinghouse.
